Small Steps, Big Vision - Grab and Go Guide to support the realisation of the Ambitions for Palliative and End of Life Care.
The Grab and Go Guide is a resource based on research conducted at The Open University. The Framework is composed of six Ambitions and eight Foundations. People told us during the research that they would value a resource to aid understanding and guidance on implementation of the Foundations.Therefore the Guide was developed to aid facilitation of discussions and reflections within teams and among partners and across systems about the Foundations and how they could be implemented to support realisation of the vision of the Ambitions Framework to improve care. In the guide each Foundation has a descriptor, prompts to stimulate thinking and examples of the foundaton in action/practice that will be benefical to learners and others to read about and to try out in their context - to help them identify areas theycan act on. Thus, the guide serves asa starting block to turn small steps into the big vision.

The Grab and Go Guide is based on research conducted at The Open University. The resource was developed to support discussions and reflections about the Foundations of the Ambitions Framework to support implementation and a vision of improved care.
