CLEAR Living Well Toolkit v1.0
This CLEAR toolkit is designed to support you with the adaptation and implementation of new models of care in Primary Care settings to provide an improved Cardiovascular Disease (CVD) prevention service. These seven models of care were developed as part of the CLEAR Living Well programme in partnership with the three Primary Care Networks (PCNs) from CLEAR Living Well 2023.
The CLEAR Living Well toolkit is primarily aimed at PCNs who feel they are mature in their approach to workforce and pathway redesign or quality improvement.Those PCNs who feel less confident may still benefit from looking at the new models of care and change methodology to create visions for the future.
The toolkit may also be useful for Integrated Care Systems (ICSs) where they have identified CVD issues and are looking for potential solutions that can either be undertaken by their PCNs or as a system wide project.
This toolkit will guide you through the preparations required for transformation work, give you the essential tools to implement a choice of seven different models of care to improve CVD prevention services for different cohorts and outline some key next steps.
