Investigations Training 1
By the end of the session participants will be able to -
1. Identify the different types of investigation undertaken within LCHS.
2. Grade an incident, plan a multidisciplinary review, identify sources of evidence and tools used to undertake an investigation.
3. Be able to apply the principles of the 'Being Open' policy.
4. Recognise how the principles and concepts used in aviation are mirrored in healthcare.
5. Undertake an investigation, identify lessons learnt, write an action plan and understand how these are monitored and shared.
6. Use A Just Culture Guide to determine a fair and consistent course of action towards staff involved in patient safety incidents, recognising the importance of human factors within errors.
7. To feel confident to involve complainants, patients and the public to work with LCHS through each investigation and after it has been completed to help us to improve our services.
