Medicines reconciliation has been defined by the Institute for Healthcare Improvement as the process of obtaining an up-to-date and accurate medication list that has been compared to the most recently available information and has documented any discrepancies, changes, deletions or additions, resulting in the end product, “a completed list of medicines, accurately communicated”.

Within the hospital setting, this is most obviously applicable to the period around admission, when it is important that healthcare professionals have access to accurate and comprehensive information regarding a patient’s usual medication and associated factors that may have an impact on subsequent treatment.

This Module covers:

  • The accurate completion, verification and recording of medication histories
  • Reconciling the verified medication history with the most recent prescription available and referring discrepancies to a pharmacist or medicines reconciliationtechnician
  • Recognising own limitations
  • Reading and understanding patient’s notes

Additional information

This module provides full details of the training and assessment process for pharmacy staff in order to gain regional accreditation for medicines reconciliation.
