Medicines optimisation training to support discharge facilitation is required to provide an effective and safe medicines service to the patient and the multidisciplinary team involved in the discharge planning process. Pharmacy staff will need to work with the discharge facilitator/ nurse/ doctor and on-going care team, as appropriate, to prepare the discharge medicines, check patient needs and prepare discharge documentation appropriate to the patient’s needs and destination. Discharge facilitation involves a number of skills including excellent communication, medicines reconciliation, accuracy checking and patient consultation skills.

This module provides full details of the training and assessment process for pharmacy technicians to follow in order to gain regional accreditation for discharge facilitation.
