TEMPLATE AGREEMENT for multi-sector rotations

These documents are provided as a tool to assist foundation trainee pharmacist training site providers to put in place a rotation agreement between the employer of a trainee pharmacist and the provider of a rotation. Use of these template agreements is not mandatory.

The template Rotation Agreement and the template agreement for use by Rotation Providers with foundation trainee pharmacists who are going to be joining them on Rotation (“the Templates”) are provided by NHS England as example agreements, to assist Employers and Rotation Providers with arranging Rotations.  NHS England does not require the Templates to be used by Employers and/or Rotation Providers.  The Templates are provided as an example of the sort of agreement that may be appropriate.  The Templates are provided ‘as is’, without any representation or endorsement made and without warranty of any kind whether express or implied, including but not limited to the implied warranties of satisfactory quality, fitness for a particular purpose, accuracy or completeness.  NHS England will not be liable for any decision made or action taken (or not taken) in reliance on the information contained in the Templates.
