Qualified clinical-staff are considered active healthcare professionals and have a responsibility to make sure that patient and public safety is paramount. They are required to work within the limits of their competence, exercising professional ‘duty of candour’ and raising concerns immediately whenever they come across situations that put patients or public safety at risk.

This form is to be used by qualified clinical-staff to self-declare that they are competent to practice the specified procedure. Digitally signing this agreement does not remove your responsibility for maintaining and updating your clinical competencies as described in NMC Code (2018)
