To record a ‘Tracer’ which shows the location of the patients' historic paper casenotes, which may be required by clinicians when reviewing historic patient data recorded on paper prior to Epic.

Recording accurate Tracers is essential and enables ease of locating casenotes by Trust staff and the Health Records team (including in an emergency).
