Small Steps, Big Vision - Grab and Go Customisable Template to support the realisation of the Ambitions for Palliative and End of Life Care
This Grab-and-Go guide builds on research about how people use the Ambitions for Palliative and End of Life Care framework. This version of the guide is a customisable template to enable you to use your own prompts and list your own examples of action. We recommend using Adobe Acrobat Reader to fill it, which free: https://get.adobe.com/uk/reader/
The research was conducted by a team at The Open University. The research examined case studies and uncovered what helps and hinders people to realise the Ambitions. We found a wide range of practices and confidence levels when it came to using the framework, with many people not knowing where to start or how to progress. A focus on foundations enabled people to work towards multiple Ambitions. Thus this customisable guide serves as a starting block for youto turn small steps into the big vision.
