Healthcare systems can be overwhelming and difficult to manage for our patients who can often feel disempowered and unable to participate in their own care. Personalised approaches to care can support our patients to feel more in control and empowered to work in partnership with healthcare professionals.

This eLearning introduces the key concepts of personalised care and is for all staff who are interested in working with individuals in a personalised way to improve health and wellbeing and wish to gain confidence in facilitating and promoting personalised conversations that enable us to identify what matters to our patients and focus the support we offer on what is important to them.

Reflecting on what matters to us and what matters to our patients can positively impact on our own health and wellbeing and help us to better understand and improve the experience of our patients. This is an interactive package during which you will be encouraged to reflect on your own experiences, thoughts and ideas.
