Medicines reconciliation training is separated into two parts:

  • Level 1 - Accurately complete, verify and record medication histories (module 3), and
  • Level 2 - Reconcile the verified medication history with the most recent prescription available (module 4)

This Module covers:

  • The accurate completion, verification and recording of medication histories
  • Reconciling the verified medication history with the most recent prescription available and resolving discrepancies working within own competency and role
  • Recognising own limitations
  • Reading and understanding patient’s notes
  • Basic clinical and technical accuracy checks on ward prescriptions
  • Advising patients on the use of their medicines
  • Underpinning therapeutic knowledge

Additional information

This module provides full details of the training and assessment process for pharmacy technicians in order to gain regional accreditation for medicines reconciliation (level 2).
