The Yorkshire and Humber Care Record (YHCR)

The YHCR allows clinical staff to access and share up-to-date clinical information held at multiple health and care settings across the Yorkshire and Humber Care Region. These records are embedded within TRFT Sepia which is accessed from Meditech, SystmOne or as a standalone tool. The YHCR shares information on patient demographics, documents, allergies, encounters, appointments, conditions, and procedures. This information can be viewed at any time to help inform our patient care planning.
