PRosPer – Long-Term Conditions
The PRosPer Long-Term Conditions programme provides training in personalised care, prehabilitation and rehabilitation for people living with long-term conditions (LTCs). As multimorbidity increases across England, this programme supports the workforce to deliver care that is coordinated around what matters to individuals and tailored to their strengths and needs.
The programme currently includes multiple elearning sessions, offering foundation, intermediate, and a newly added strategic-level module. These sessions are designed to help learners apply personalised care and support approaches across clinical practice and service development.
The programme is aimed at:
- Health and care professionals working with people living with long-term conditions (LTCs)
- Multidisciplinary teams, particularly allied health professionals (AHPs)
- Clinical leads, commissioners and decision-makers involved in designing or delivering prehabilitation, rehabilitation and personalised care pathways
Key areas covered
- Core principles underpinning prehabilitation: screening, assessment and intervention planning for people with LTCs
- How rehabilitation interventions address symptoms and needs related to LTCs
- The role of MDTs, with a focus on AHP contributions in personalised, proactive care
- Designing and delivering personalised care and support planning
- Applying improvement approaches, including at a strategic level, to enhance local services
Learning objectives
By completing the programme, learners will be able to:
- Understand the theoretical foundations of prehabilitation and rehabilitation for people with LTCs
- Recognise how personalised care supports improved outcomes for people with single or multiple LTCs
- Apply structured approaches to screening, assessment, and intervention planning
- Identify how MDTs collaborate to provide person-centred support
- Use tools and information from the strategic-level session to implement small improvement projects or contribute to wider service transformation
