SBAR Communication Training for Primary Care and Community Teams
Everyone communicates, but how well do we do this practically? The SBAR communication tool is a structured tool to support the effective handover of information. It can be used both verbally (e.g. in-person meeting, handover, MDT, telephone) and in writing (e.g. emails, letters). We have put together two training packages which focus on the handing over of patient information in the context of Community Teams and Primary Care.
Objectives: By the end of the training, you will be able to explain what SBAR is, when and how to use it and have a chance to put SBAR into practice using case studies relevant to the setting. You can also use our self-reflection worksheet to reflect on your learning.
Target Audience:
- Anyone working in the context of a Community Team, e.g. community nurses, urgent response, specialist community services.
- Anyone working in Primary Care (including clinical and non-clinical), e.g. GPs / partners, nurses, AHPs, ARRS roles (such as social prescribers and care coordinators), admin staff.
