Annual Health Checks for Primary Care

Welcome to the Learning Disabilities Annual Health Check Learning Hub for people who work in Primary Care. 

The purpose of this hub is to provide links to training and learning opportunities regarding delivery of Annual Health Checks. 

We recognise that there are many training and learning opportunities, but the aim of the hub is to signpost primary care staff to those which will hopefully be of most benefit and in a way that covers the main themes. 

We have organised the hub into 2 main categories. 

These are:   

1. Communication and interactions related to Annual Health Checks. This includes: 

  • Training around reasonable adjustments 
  • Basic awareness training relating to people with a learning disability 
  • Understanding what a learning disability is 
  • Simple communication training 

  

2. Specific learning aspects related to delivering Annual Health Checks. This includes:   

  • Understanding what an Annual Health Check is 
  • Understanding why it is important 
  • Getting the best out of an annual health check 
  • Understanding the elements of an annual health check 
  • Roles and responsibilities associated with an annual health check 
  • Health Action Plans, why they are important
